This position is dedicated to the Transplant program and is required to support appropriate assessment and follow up for patients. Provides direct psychological, social and practical care to a select caseload of patients evaluated for transplant. Completion of assessments, referrals to outside agencies and coordination of care. Works collaboratively with the transplant service teams and inpatient multidisciplinary staff. Participation and attendance at selection meetings and follow up with patients for inpatient and outpatient related to medication adherence, access to community resources, financial assistance, crisis management, adjustment to illness.
Full Time, Day Shift
8:30 am- 5:00 pm
Weekend coverage is paid but on a volunteer basis. Staff are encouraged to sign up for available weekend assignments.
Location: JHH, Carnegie 100
Education: Master's degree from an accredited school of social work
Experience: 1 year related experience in clinical social work. Internship may apply
Licensure: LGSW or LCSW-c
Working Condition: Works in patient care areas where there are no physical discomforts due to dust, dirt, temperature and the like
Pay Grade: CJ
